Our team within Carer Services is currently seeking a Specialist Transition Officer to assist in securing housing and wrap around support to transition young people from residential care and into independent living arrangements. This role requires undertaking tasks that provide assistance with placement transition work in relation to scoping, documenting and coordinating for improved placement outcomes for young people in partnership with a broad range of stakeholders.
As part of our team, you will have the opportunity to contribute by:
- Undertaking transition work that secures housing, wrap around supports and improved outcomes for placement.
- Developing and maintaining professional working relationships with a broad range of stakeholders.
- Keeping case files well organised, up to date and be able to provide concise and accurate information regarding service position.
- Developing and sharing knowledge with DCP Case Management regarding housing pathways and services through collaborative consultation.
- Providing culturally appropriate practices for Aboriginal children and children from diverse cultural backgrounds.
To be successful in this role, you will have:
- Expertise in managing workloads, organising and planning work activities whilst achieving program outcomes within required timeframes.
- Experience in assessing the needs of young people in the context of securing housing, wrap around services and improved outcomes.
- Capacity to coordinate and manage stakeholder meetings to successfully secure housing and appropriate services.
- Exceptional interpersonal skills, including advocacy and negotiation skills, supported by a high level of written assessment capabilities.
- Knowledge and understanding of child development, parenting capacity and environmental factors to assess the needs of young people.
